Dear Parent or Guardian,
As you know, Darrell Lynn Hines Academy (DLH Academy) is chartered by the City of Milwaukee and monitored by the city’s Charter School Review Committee (CSRC). As a charter school, DLH Academy seeks to provide a quality educational program for your child.
CSRC has contracted with the NCCD Children’s Research Center (CRC)* to monitor the educational program/academic outcomes for children enrolled in your child’s school. CRC is requesting your help in obtaining information about the quality of DLH Academy. With your input, CRC will be able to provide a comprehensive assessment of your child’s learning experience during the 2015-2016 school year.
This year, we are excited to offer an online version of the survey on the school’s website. Simply type this URL (https://secure.vanguardsw.com/survey/v2/survey2.dsb?ID=6451698842) into the web browser on your phone, computer, or tablet. Once the survey is open, follow the instructions provided.
All information received from individuals by CRC is kept strictly confidential. The information will be incorporated into the annual monitoring report without any individual references or identification.
